About the Role
The role will include both in-clinic and home-based service delivery in addition to running hydrotherapy and land-based group exercise. We provide supports across NDIS, Aged Care, Medicare, Hi-Caps and Private Services, with a strong emphasis on participant choice, dignity and independence.
You will deliver evidence-based interventions that align with each client’s goals, support plans and functional capacity, while working collaboratively within a multidisciplinary team to ensure coordinated and high-quality care.
Key Responsibilities

Deliver person-centered, goal-oriented therapy in line with client support plans and the NDIS/Aged Care Practice Standards

Conduct comprehensive assessments and develop individualised, evidence-based treatment and exercise programs

Support clients to build functional capacity, independence and participation in daily life

Work collaboratively with internal teams, support coordinators and external stakeholders

Maintain accurate, timely and compliant clinical documentation and progress reports

Identify and manage risks, incidents and restrictive practice considerations in accordance with organisational and NDIS/Aged Care requirements

Participate in clinical supervision, team meetings, quality improvement activities and professional development
