Care Manager - Rockhampton

About the role

Mercy Community Aged Care Services are on the lookout for an experienced Care Manager (Full Time Permanent) to join our team in Rockhampton! The purpose of this role is to support the Facility Manager in the clinical management of health and clinical care outcomes of the residents in the facility. Pay level for this role is negotiable depending on experience and qualification.

 The duties of the role entails but not limited to:

  • Provide supervision to clinical staff to ensure evidence-based practice is implemented
  • Oversee the implementation of person-centred care and provide leadership to the team
  • Document and report on clinical and service outcomes
  • Oversee and ensure that infection control principles and procedures are maintained
  • Facilitate communication between staff and resident/representative to support partnering in care
  • Maintain working relationships with all care staff, medical practitioners and allied health professionals, ensuring documentation supports and optimises the Aged Care Funding Instrument (AN-ACC)

 About you

To be successful in the role, you will possess:

  • Registered Nurse with AHPRA registration
  • Prior experience in aged care or an ability to demonstrate relevant experience
  • Experience in leadership roles or opportunities/skills
  • Ability to assess, plan, deliver and evaluate care for residents
  • Current valid Drivers Licence
