Pearl Home Care Gladstone is looking for a Full-time Care Partner or Clinical Care Partner to coordinate Support at Home packages.
The Care partner provides relationship-based care coordination, engagement, and service monitoring for Pearl Home Care clients receiving home care. Acts as a contact for clients and families, supporting service delivery, budget use, and client satisfaction under the Support at Home Program. Liaise with others in the leadership team to ensure the delivery of high-quality services.
Key Responsibilities
Client Engagement and Support
· Build and maintain positive client and family relationships.
· Provide information on service options, schedules, and budgets.
· Support independence, dignity, and choice.
Service Co-ordination
· Coordinate and undertake rostering and scheduling of support services.
· Monitor delivery of non-clinical care and address issues promptly.
· Track utilisation of Support at Home budget.
Case Monitoring and Escalation
· Monitor client progress and goals.
· Identify changing needs and escalate to Clinical Care Partner as required.
· Maintain accurate documentation in client management system.
Quality and Compliance
· Ensure compliance with Aged Care Quality Standards and policies.
· Participate in client feedback processes and continuous improvement.
· Participate in audits, incident reviews, complaints and continuous improvement activities.
Knowledge, Experience and Skills

A degree or diploma in Nursing or a Diploma in Community Services.

A minimum of 2 years of experience in a similar role is preferable

Understanding of relevant legislation, policies and guidelines governing the provision of home care services, such as the Aged Care Quality Standards, Home Care Common Standards, and the National Disability Insurance Scheme.

Demonstrated advanced computer skills including Microsoft Office 365 and various other software applications.
Probity Requirements

A National Criminal Clearance Check less than 6 months old prior to engagement: and subsequent criminal records checks at least once every three years.

Working with Children Check or willing to obtain.

NDIS Screening check or willing to obtain.

CPR and First Aid Certificate or willing to obtain.

The Right to Work in Australia.
