Are you passionate about supporting older Australians to remain independent in their own homes? We’re looking for a compassionate and organised Care Partner to join our growing team for an immediate start.

✨ About the Role
As a Care Partner, you will:
• Build strong relationships with clients and families
• Coordinate services and support schedules
• Monitor client wellbeing and goals
• Assist with Support at Home budgets and service delivery
• Work closely with our Clinical Team to ensure high-quality care

📍 Based in Gladstone with travel throughout the local region.

✅ What We’re Looking For
• Experience in aged care, disability or community services
• Excellent communication and organisational skills
• Ability to manage multiple priorities in a fast-paced environment
• Strong computer skills and experience with client management systems
• Current Driver’s Licence & Police Check/NDIS Screening
