About the Organisation:
Gladstone Region Aboriginal and Islander Community Controlled Health Service Limited (“GRAICCHS”) trading as Nhulundu Health Service is an Aboriginal Community Controlled Health Service operating in Gladstone and the greater Central Queensland Region delivering a comprehensive range of primary health care, aged care and community support service functions.

About the Role:
The purpose of the Case Manager, Next Step Plus (NSP) and Extended Post Care Support (EPCS) is to provide a localised collaborative approach engaging and supporting youth people from the age of 15, to successfully transition from care.
As a Case Manager - Next Step Plus and Extended Post Care Support, you will help develop the skills and knowledge they need to live as an independent adult; through delivering available funding to support young person’s needs and goals and ensure those who have left care are able to receive additional support and until they are 25, if required.
More specifically this role will focus on key activities, including but not limited to:
- Building knowledge, strength and capacity of individuals and their families;
- Managing a case load of clients with varying needs and identified issues;
- Building individual capability to participate in and lead the coordination of their own supports and goals;
- Focusing on young adults not engaged in formal supports when leaving case, through assertive outreach;
- Providing more intensive and proactive support to young adults, particularly through developing and maintaining connection to family, culture, country and community;
- Developing networks and building community support for promotion and participation in services;
- Collaborating with Child Safety staff to support young people in care (over the age of 15), in the development of a Transition and Financial plan to gains the skills, knowledge, and networks they need including identifying a natural mentor;
- Linking young adults to opportunities that promote independence
This is a five (5) month – Maximum-Term Full-Time Contract based in Gladstone and Biloela
About You:
To be successful in this role, we are looking or an individual which obtains the following:
- Bachelor qualifications in Counselling, Psychology, Social Work, Occupational Therapy, Nursing or Mental Health; and relevant experience in mental health assessment;
- Experience with the child protection system;
- Ability to communicate with Aboriginal and Torres Strait Islander people effectively and with sensitivity;
- Ability to work outside of your ordinary hours and some travel across the region will be required;
- Ability to meet Child Safety and Personal History Screening (LCS2) requirements;
- National Police Clearance must be attained, and declaration made of any future charges or convictions that could potentially affect your role;
- Current Blue Card (working with children and young people) or willingness to apply for a Blue Card;
- Current C Class Drivers Licence (Qld) – essential.
- Current First Aid and CPR.
