About the role

As a Clinical Care Coordinator, you’ll work alongside the Clinical Lead and Operations Manager to deliver essential services across the Country to Coast Queensland (CCQ) region. This is a full-time role on a maximum term contract until 30/06/2026. This will be based in Rockhampton on Darumbal country land (part-time may be considered for the right candidate).

 
Your responsibilities will include:

  • Conduct assessments and develop case formulations and treatment plans
  • Providing psychological interventions
  • Manage a clinical caseload and provide direct support
  • Conduct risk assessments and develop management plans
  • Adhere to confidentiality, ethics, and continuous quality improvement standards
  • Engage in data collection and contribute to monitoring and evaluation
  • Collaborate with multidisciplinary teams and external stakeholders
  • Facilitate care planning and review processes
  • Monitor and escalate mental health needs as necessary

Requirements, Experience and Qualifications

  • Open driver licence
  • Undergo a current or satisfactory Police Check
  • Working with children check (Blue card) or ability to obtain
  • Flexibility to travel and work after hours
  • Passionate about making a difference in the mental health sector
  • Experience with complex mental health presentations
  • Experience working in cross-cultural settings, particularly with First Nations people
  • Experience in trauma-informed and recovery-oriented services
  • Strong stakeholder engagement, organisational, and communication skills
  • Ability to manage multiple tasks, navigate challenging situations, and work within a team
  • Tertiary qualification in psychology, social work, mental health nursing, or a relevant discipline
  • Current annual practicing certificate from AHPRA
