This is an exciting opportunity for a motivated clinician who enjoys combining hands-on clinical care with care coordination, case management, and leadership within the community aged care sector.

About the Role

As a Clinical Care Partner, you will manage, coordinate, and deliver person-centred, evidence-based clinical care for clients living at home in alignment with their goals and the Support at Home Program.

You will work closely with clients, families, support workers, allied health professionals, GPs, and the broader Pearl Home Care team to ensure high-quality outcomes and continuity of care.

Key Responsibilities

βœ” Conduct holistic client assessments and develop individualised care plans
βœ” Monitor client wellbeing and identify/escalate clinical concerns
βœ” Coordinate services and multidisciplinary supports
βœ” Support hospital discharge planning and transitions of care
βœ” Deliver clinical interventions within scope of practice
βœ” Maintain accurate clinical documentation and case notes
βœ” Monitor Support at Home budgets and service utilisation
βœ” Provide mentoring and clinical guidance to support staff
βœ” Participate in quality improvement, audits, and compliance activities

About You

  • A strong communicator with excellent relationship-building skills
  • Passionate about aged care and improving quality of life for older Australians

Essential Requirements

βœ” Current AHPRA registration (RN or EN)
βœ” Experience in aged care and/or community nursing
βœ” Current First Aid & CPR
βœ” Driver’s Licence and reliable vehicle
βœ” National Police Check and/or NDIS Worker Screening
βœ” Strong communication and documentation skills
βœ” Ability to coordinate and delegate care effectively

Desirable

⭐ Experience in Support at Home or Home Care Packages
⭐ Care management or clinical coordination experience
⭐ Knowledge of aged care reforms and Aged Care Quality Standards
⭐ Experience working with multidisciplinary teams
⭐ Advanced computer and documentation skills
