About the Role
Working under the direction of our Registered Nurses and Clinical Care Partners, you will deliver evidence-based clinical care and support to clients across Gladstone and surrounding areas.
This community-based role offers variety, autonomy, and the opportunity to build meaningful relationships with clients and families while supporting them to remain safe and independent at home.
Key Responsibilities
✔ Deliver safe, person-centred nursing care in line with care plans
✔ Conduct clinical observations and monitor client wellbeing
✔ Assist with wound care, continence care, chronic disease management, and medication support
✔ Report changes in client condition to the RN promptly
✔ Maintain accurate clinical documentation and case notes
✔ Participate in care reviews and multidisciplinary collaboration
✔ Promote dignity, independence, wellness, and reablement
✔ Support compliance with Aged Care Quality Standards and organisational policies
About You
We are looking for someone who:
- Has a genuine passion for aged and community care
- Enjoys building strong relationships with clients and families
- Works well independently and as part of a team
- Demonstrates strong clinical judgement and communication skills
- Is professional, compassionate, and organised
Essential Requirements
✔ Current AHPRA registration as an Enrolled Nurse
✔ Diploma of Nursing qualification (or equivalent)
✔ Current First Aid & CPR
✔ Current Driver’s Licence and reliable vehicle
✔ NDIS Worker Screening Clearance
✔ Strong interpersonal and communication skills
✔ Demonstrated experience working with older adults
✔ Sound computer and documentation skills
Desirable
⭐ Experience in community, aged care, or hospital nursing
⭐ Experience using client management systems such as Visual Care
⭐ Understanding of the Support at Home Program and aged care reforms
⭐ Minimum 1 year nursing experience preferred
