About the role
Make a real difference in the lives of people and families across North Queensland. As the Northern Queensland Primary Health Network (NQPHN) Greater Choice for At Home Palliative Care Manager, you will lead the design and delivery of programs that help people receive compassionate, high-quality palliative care at home—where most prefer to be.
In this rewarding role, you will:

improve access to community-based palliative care

support primary health and community care providers

help reduce unnecessary hospital visits by ensuring care is delivered at the right time and in the right place.
The successful candidate will:

Manage the Greater Choice for At Home Palliative Care program deliverables aligned with NQPHN’s strategic objectives and operational plan.

Map existing palliative care and aged care services across the NQPHN region.
-
Map existing education and training resources relevant to the program, including those through Palliative Care Australia and National Palliative Care Projects.

Plan, develop, and coordinate activities to improve the coordination, integration, and continuity of care at the aged care and primary health care interface.

Work collaboratively with NQPHN’s Health System Integration and Innovation team and Data, Information and Digital team to undertake a needs assessment to plan and inform the development of aged care programs.

Plan, develop, and implement activities to increase awareness of, facilitate, and coordinate access to, safe quality palliative and end-of-life care at home.

Plan and coordinate relevant educational events.

Support co-design and referral pathways, as relevant.

Establish and support working groups and networks.

Improve coordination of care for patients across health care providers and the integration of palliative care services across the NQPHN region.
Key selection criteria
To be successful in this role, you will have:
-
A degree in a health-related discipline and/or five years’ experience in a similar role, or alternatively, a VET qualification and five years relevant experience.

Demonstrated understanding of the health challenges of older persons and the opportunities and challenges for the palliative care sector in the North Queensland region.

Demonstrated understanding of health commissioning processes, strategic health service design, and delivery in a community environment.

Demonstrated project management experience, systems thinking, and a high-level ability to conduct cohesive networking and stakeholder engagement with the health sector across the NQPHN region.

Demonstrated experience in leading, developing, and contributing to a multi-disciplinary and multifaceted team across geographical locations.

Demonstrated interpersonal, communication (oral and written), and conflict and negotiation skills.

Demonstrated ability to work with diverse communities, including Aboriginal and/or Torres Strait Islander and Australian South Sea Islander peoples in our region.
