NDIS Support Coordinator

About the role

This is a multi-faceted role with the successful candidate responsible for:

  • Assisting existing and new participants to implement their NDIS Plans and coordinate their services and supports
  • Building and maintaining relationships with mainstream and disability services providers and actioning of appropriate referrals 
  • Assisting with plan reviews and reporting on progress towards goals
  • Meeting a weekly billing KPI
  • Building NDIS participant and family capacity to understand and navigate service systems
  • Regularly monitor expenditure and support participants to remain informed as to their rate of expenditure and the potential effects

About you

The following skills/attributes will be viewed favourably:

  • Previous experience in community services, disability employment services, mental health services or similar
  • Completed tertiary qualifications in Social Work or Community Services is desirable
  • Assisting clients and their formal supports (families/carers/guardians) to understand their NDIS plan and associated processes
  • Engaging and coordinating service providers on behalf of clients and linking clients to relevant providers and services in accordance with individual NDIS plans
  • Excellent oral and written communication skills and experience writing reports and analysing data.
  • Familiarity & strong connections with disability, mainstream and community networks

Essential criteria:

  • Hold a current: Blue Card, Worker Screening/Yellow Card or willing to obtain
  • Current First Aid Certificate or willing to obtain
  • Full QLD Driver's License & Own car with at least third-party insurance
