NDIS Support Coordinator

About the role
This is a multi-faceted role with the successful candidate responsible for:
- Assisting existing and new participants to implement their NDIS Plans and coordinate their services and supports
- Building and maintaining relationships with mainstream and disability services providers and actioning of appropriate referrals
- Assisting with plan reviews and reporting on progress towards goals
- Meeting a weekly billing KPI
- Building NDIS participant and family capacity to understand and navigate service systems
- Regularly monitor expenditure and support participants to remain informed as to their rate of expenditure and the potential effects

About you
The following skills/attributes will be viewed favourably:
- Previous experience in community services, disability employment services, mental health services or similar
- Completed tertiary qualifications in Social Work or Community Services is desirable
- Assisting clients and their formal supports (families/carers/guardians) to understand their NDIS plan and associated processes
- Engaging and coordinating service providers on behalf of clients and linking clients to relevant providers and services in accordance with individual NDIS plans
- Excellent oral and written communication skills and experience writing reports and analysing data.
- Familiarity & strong connections with disability, mainstream and community networks
Essential criteria:
- Hold a current: Blue Card, Worker Screening/Yellow Card or willing to obtain
- Current First Aid Certificate or willing to obtain
- Full QLD Driver's License & Own car with at least third-party insurance
